Children who need skilled nursing or personal care at home — tracheostomy or ventilator dependence, seizure disorders, G-tube feeding, complex respiratory needs, cerebral palsy, muscular dystrophy, spina bifida.
Seniors Needing Placement
Discharge planning support for a senior who needs a personal care home, assisted living, memory care, or skilled nursing facility — free to families, placements often within 24–72 hours.
Georgia Medicaid Enrolled
Children with Georgia Medicaid, including the CMOs — Traditional Georgia Medicaid, Amerigroup, CareSource, Peach State Health Plan. Ages 5–21, or under 5 with skilled home nursing and current physician-ordered services.
Discharge & Step-Down
NICU/PICU discharges, hospital step-down, and patients transitioning from a facility to home or to a new facility who need continued care to stay safe.
